Objectives To identify clinical features of aquaporin 4 (AQP4) antibody positive optic neuritis (ON) in neuromyelitis optica (NMO) and NMO spectrum disorder (NMOSD) a UK cohort. Methods We performed retrospective review case notes NMOSD patients referred to the joint National service (based at Liverpool Oxford) for AQP4 by cell based assay between 2006 2011. The features, visual acuity, perimetry, optical coherence tomography results evoked responses were analysed. Results 50 identified. neuronopathy was associated with severe sight impairment. However some had mild ON indistinguishable clinically from MS. Patients isolated longitudinally extensive myelitis (LETM) antibodies did not show subclinical nerve abnormalities. Conclusions is expressed generally causes damage. In those without do cause asymptomatic evidence damage unlike Multiple sclerosis which commonly involvement. factors that influence whether require elucidating.
